Boulder outpatient health care workers asked to complete survey to receive vaccine

Boulder County health officials are encouraging all outpatient health care workers providing direct care to patients to complete a vaccine notification survey that will connect providers to clinics when it is available.

The COVID-19 Outpatient Healthcare Worker Vaccination Notification survey will assist outpatient workers to receive the vaccine in the initial phase of its distribution, Boulder County Public Health said Tuesday.

“We are partnering with the Boulder County and Longmont offices of emergency management to coordinate vaccine clinics for these groups,” said Indira Gujral, the division manager of Boulder County Public Health Communicable Disease and Emergency Management.

When more dosages of the COVID-19 vaccine become readily available during Phase 2 and Phase 3, these workers will be invited to enlist in the U.S. Centers for Disease Control and Prevention’s program so they will be eligible to administer the vaccine.

Although outpatient workers will be asked to join the program, invitation to enroll will be based on the critical populations each providers serve and how it fits within Colorado’s Phased Implementation Plan, officials said.

For an outpatient worker to become eligible, they must be credentialed or licensed in the state to possess or administer vaccines or services, sign and agree to conditions in the CDC COVID-19 vaccination program provider agreement, and complete the CDC’s form for each location they will administer the vaccine.

Those within Colorado interested in becoming a provider must fill out a from by the Colorado Department of Public Health and Environment.
